Ashley Molina, M.D., Inc., doing business as Vigorize Health (“Vigorize Health,” “we,” “us,” or “our”) operates the website located at vigorizehealth.com (the “Website”). This Privacy Policy explains how we collect, use, disclose, and protect information when you visit the Website, submit a form, book an appointment online, or otherwise interact with us online.

1. Your Medical Information and Our Notice of Privacy Practices

Vigorize Health is a medical practice. Once you become a patient, the health information we create or receive in the course of your care (for example, your medical history, lab results, treatment plans, and billing records) is “protected health information” under the Health Insurance Portability and Accountability Act (“HIPAA”) and “medical information” under the California Confidentiality of Medical Information Act (“CMIA”). That information is governed by our Notice of Privacy Practices, which is available [at our office / at LINK] and describes your rights regarding your health records.

This Privacy Policy covers information collected through the Website generally. If there is a conflict between this Privacy Policy and our Notice of Privacy Practices regarding your protected health information, the Notice of Privacy Practices controls.
